Health Information:
- Every incoming freshman MUST have a comprehensive physical and provide proof of immunizations in order to be enrolled and begin September classes in school.
- Every Student Athlete (all grade levels) must have a yearly physical valid for 12 months. For example, if a student had a physical in March 2023, it is valid only until March 2024. In order to participate in a sport, a student-athlete must have a new physical if 12 months since the last physical have passed.
- Every Student-Athlete must complete the NJ State Health History Update Questionnaire (HHQ) form, be signed by a parent/guardian and submitted to Holy Spirit High School- only if the most recent physical was administered more than 90 days prior to the first official practice of a new sport season in order for a student to participate.
