Transcript Request

To obtain an official copy of your transcript, you must provide a request for release of records to the Guidance Office at Holy Spirit High School by completing the Transcript Request Form.

You may also choose to send a signed letter which should indicate your name at the time you graduated (maiden name), the year of graduation/withdrawal from Holy Spirit High School, the address(es) to which you would like your transcript sent and your current contact information.

Please include the $5.00 per transcript fee for each request with the letter/form, in cash, money order, or check, made payable to Holy Spirit High School.
