- Official academic records for 6th, 7th and 8th grades
- Standardized test results
- Child study team (if applicable)
- Official discipline record
- Teacher recommendation
- Completed Application
- HSPT completed
Complete either the online application or the paper application and return to the admissions office at Holy Spirit High School.
Have parent sign the records release form and submit to middle school.
This form will allow HSHS to obtain the following:
- 6th,7th and 8th Official Academic Records
- Standardized Test Results
- Official Discipline Records
- Child Study Team (If Applicable)
One teacher recommendation is required for each applicant.

There is a mandatory registration fee of $250 for each registrar.

A preliminary round of acceptance letters will be sent in mid-December for all students who have applied and registered before November 30, 2019. After the preliminary round of letters has been sent, acceptance letters will be sent on a rolling basis. Once all of your application materials have been received and reviewed, Acceptance Notification Letters are mailed.
